✔️ Most people tolerate Rovast 10 mg well, but some may experience:

• Headache
• Muscle pain (myalgia)
• Constipation
• Fatigue (asthenia)
• Abdominal pain
• Nausea

✔️ Dosage & Administration of Rovast 10

For Pediatric Patients (10-17 years) with Heterozygous Familial Hypercholesterolemia

• Standard dosage: 5-20 mg per day
• Maximum dose: 20 mg/day (higher doses are not studied in this age group).
• Dosage should be tailored based on therapeutic goals, with adjustments made at 4-week intervals or longer.

For Patients with Severe Cholesterol Levels or High Risk of Heart Attack/Stroke

• 40 mg once daily is recommended.

For Patients with Homozygous Familial Hypercholesterolemia

• Starting dose: 20 mg once daily
• Response to treatment should be based on pre-apheresis LDL-C levels.

Dosage Considerations for Asian Patients

• It is recommended to start at 5 mg once daily due to increased sensitivity to the drug.

For Patients Taking Specific Medications

• With Cyclosporine: Limit Rosuvastatin dose to 5 mg once daily.
• With Lopinavir/Ritonavir or Atazanavir/Ritonavir: Maximum dose should not exceed 10 mg once daily.

Concurrent Use with Other Lipid-Lowering Medications

• Using Rosuvastatin with Niacin or Fenofibrate may increase the risk of muscle-related side effects—dosage adjustments may be required.
• Gemfibrozil should be avoided, but if used together, the Rosuvastatin dose should not exceed 10 mg daily.

For Patients with Severe Kidney Impairment (ClCr <30 mL/min/1.73m²)

• Starting dose: 5 mg once daily
• Maximum dose: 10 mg daily (if not on dialysis).

✔️ Significant drug interactions include:

• Cyclosporine: Increases Rosuvastatin concentration. Limit dose to 5 mg/day.
• Gemfibrozil: This should be avoided due to increased Rosuvastatin exposure. If necessary, limit the dose to 10 mg/day.
• Lopinavir/Ritonavir or Atazanavir/Ritonavir: Enhances Rosuvastatin levels; do not exceed 10 mg/day.
• Coumarin anticoagulants (e.g., Warfarin): Increases INR (blood clotting time). INR levels should be monitored regularly when starting or adjusting Rosuvastatin therapy.
• Other lipid-lowering drugs (Niacin, Fibrates): Can heighten muscle-related risks.

✔️ Rosuvastatin should not be used in patients with:

• Known allergic reactions to any components of the drug.
• Liver disease, including unexplained persistent liver enzyme elevations.
• Pregnancy or women who may become pregnant.
• Breastfeeding mothers.

✔️ Use During Pregnancy & Lactation

• Pregnancy: This drug should only be prescribed to women of childbearing age if pregnancy is highly unlikely. If pregnancy occurs, discontinue use immediately.
• Lactation: While it is unclear if Rosuvastatin is excreted in human milk, similar drugs do pass into breast milk. Avoid use while breastfeeding.

✔️ Precautions & Warnings

• Use with caution in elderly patients (above 65 years), those with untreated hypothyroidism, or renal impairment due to an increased risk of muscle-related complications.
• Combining with fibrates, niacin, cyclosporine, or certain antivirals (Lopinavir/Ritonavir, Atazanavir/Ritonavir) increases the risk of myopathy.
• Discontinue treatment if there is a significant rise in creatine kinase (CK) levels or if muscle pain or weakness occurs.
• Temporarily stop treatment in patients experiencing serious acute conditions such as:
  • Sepsis
  • Hypotension
  • Dehydration
  • Major surgery or trauma
  • Severe metabolic or endocrine disorders
  • Electrolyte imbalances

Liver enzyme tests should be performed before starting therapy, at 12 weeks after initiation or dose increase, and periodically thereafter.
